Person Centred Care Planning (PCCP) Training
This Person Centred Care Planning (PCCP) training course equips healthcare staff and social care workers with the skills and confidence to create meaningful, individualised care plans that reflect each person’s unique needs, preferences, strengths, and goals. The session emphasises seeing people as individuals, understanding what matters most to them, and designing support that promotes autonomy, dignity, and wellbeing.
Participants will explore the principles and origins of person‑centred practice, learn how to identify barriers to effective care planning, and develop practical strategies for implementing and evaluating personalised plans. Defining person‑centred care What it means in practice and how it differs from traditional care models
Principles of a person‑centred approach Choice, dignity, independence, partnership, and respect
Origins of person‑centred care Exploring the development of person‑centred theory and practice
Benefits and barriers Identifying what supports or hinders person‑centred approaches in social care
Documentation Recording information clearly, accurately, and meaningfully
Implementing and evaluating person‑centred plans Turning plans into action, reviewing progress, and adapting support as needs change
